Transitional Planning

A social worker will help you plan for your child during the transitional years from adolescence to adulthood.

Program Information

The CYSN services you receive will end upon your child’s 19th birthday. At the age of 15 the social worker will support you to begin planning your child’s future. This might include assisting with:

  • Identifying, with your child, their goals for their future.
  • Applying for adult CLBC services
  • Applying for adult health services
  • Applying for Persons with Disabilities designation (PWD)
